Voluntary, community and social enterprise organisations can apply for grants of up to £30,000 to provide support for patients leaving hospital.

The NHS Hospital Discharge Support Fund will enable charitable, community and other not-for-profit organisations to help people in North Cumbria regain and maintain their independence at home and in the community by providing support for a four-week period after they are discharged from hospital.

Support could include:

• carer support
• non-clinical support (e.g., reminding patients to take medication)
• community transport
• shopping/handy man services
• befriending services
• advice and guidance, including access to benefit support
• general support in the home

Grants in excess of £30,000 will be considered in exceptional circumstances.

The funding should be fully spent by the 31st March 2022.

Organisations are encouraged to apply as soon as possible.
