Funding for Hospices to Improve Transitional Care for Young People (England, Wales, the Channel Islands, or the Isle of Man)

Grants of up to £40,000 are available to support adult hospices in testing approaches to improve transitional care for young people.

These grants are intended to improve the preparedness of adult hospices to provide health, social care and educational support for young adults as they move from children’s to adult’s services.

The funding, which is being provided through the Masonic Charitable Foundation Hospice Grants, and administered by Hospice UK, will fund between 10 and 15 grants totalling £400,000.

Projects could include:

  • co-ordination, support and advocacy – staff and peer led approaches during transition to adult orientated service
  • training - staff at all levels should have the skills and capabilities to support young people, as well as developing the skills and knowledge about the range of complex health conditions and their implications
  • tackling inequality, understanding the gaps in care locally and implementing a project to address these
  • trialling short/break respite care with clear plans to evaluate benefits
  • capital/refurbishment - to improve areas within the hospice building, making it more age appropriate

The funding is available to adult hospices that are members of Hospice UK, based in England, Wales, the Channel Islands, or the Isle of Man.

Only one application can be submitted per hospice.

The deadline for applications is 5pm on the 5th February 2024.
