Funding to Support Self-Management for Long-Term Health Conditions (Scotland)

Charities, CICs, community groups, and other not-for-profit organisations can apply for funding to support activities that empower individuals with long-term conditions to manage their health and well-being effectively.

The Self Management Fund, administered by the Health and Social Care Alliance Scotland and funded by the Scottish Government, supports co-produced, person-centred self-management initiatives across Scotland.

This funding round, titled “Self Management for Life”, offers two levels of grant:

  • Small grants of up to £15,000 for up to one year
  • Large grants of up to £120,000 for up to three years

It invites applications for early intervention initiatives that:

  • support people at an early stage in their journey with a long term condition, disability or unpaid caring responsibility, to develop their ability to self manage
  • reduce barriers to accessing health and social care support and services, to enable self management for people living with long term conditions, disabilities and/or unpaid carers
  • develop the self management capacity of people living with long term conditions, and/or their unpaid carers, while they wait for a specialist health or social care intervention

The deadline for applications is 9am on the 27th January 2025.
